Member Complaint

The Member Complaint Form can be used when a member would like to file a complaint for review by the Board of Directors.  To submit a complaint, please download and complete the form found HERE.  Completed Membership Complaint Form should be emailed to LPA's [email protected], which will be sent to the current Membership Director and Executive Director.
